On August 12, 2025, an employee was scraping material off a stainless steel roller on the wrapper line when their left hand was pulled into the rollers. The employee sustained a finger amputation.

An employee was making an adjustment on a trim saw machine when their right hand contacted the machine, resulting in a partial amputation of their right middle finger with a fracture. The machine was unguarded at the time of the incident after maintenance work was performed on the machine.

On February 24, 2021, an employee was hospitalized with a right ankle fracture after slipping and falling on ice in a parking lot while walking into work.

An employee was reaching to operate a lever on a hopper that had been raised by a forklift. The hopper shifted and pinched the employee's left ring finger against the forklift fork. The end of the finger was amputated.

On October 21, 2019, at approximately 10:15 p.m., an employee was emptying a hopper into a hog. He used his hand to release the bottom lever when it snapped back and caught his left middle finger, causing a partial fingertip amputation.

An employee was going to the breakroom when they tripped over a chair and fell, resulting in a hip fracture.

An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.

On May 9, 2025, an employee was using an air hose to clear debris from the courtyard area when she slipped and fell. The employee was hospitalized with closed fractures to her left fibula, tibia, and medial malleolus.

On April 24, 2025, an employee was opening a cylinder door on a hydraulic bridge to remove lumber when their left middle finger was caught between the door and the hydraulic bridge, resulting in an amputation without bone loss.

Two employees were using a plywood chop saw. The injured employee was holding one end of the saw with their right hand when the operator on the other end pressed the start button. The saw blade contacted the injured employee's right hand, resulting in multiple finger amputations when it amputated the hand above the thumb.

On January 17, 2025, at 12:15 p.m., an employee was moving 11-gauge steel sheets from a holding cart using a pendant-controlled, 1-ton jib hoist equipped with a plate clamp lifting attachment. With the plate clamp empty and open, the employee began raising the hoist. The clamp became caught on the bottom of the cart, tipping the cart in the direction of the employee. Several sheets of steel slid off the cart and struck the employee, resulting in several broken ribs, a broken left leg and ankle, a fractured kneecap, and a torn left ACL. The employee was hospitalized.

An employee was operating an air hoist to rotate a blending mill counter-clockwise. After he connected the hook to the mill, the hoist was raised and his hand was pinched between the hook block and the ring on the mill. The employee sustained a partial amputation of his right thumb.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
