An employee's glove became caught inside the end joint of a conveyor roller. His left little finger was amputated at the first joint.
An employee was in an aerial lift connecting a wire to a pipe inside the warehouse. When he raised the lift, his right arm contacted an overhead rotating ceiling fan blade causing a laceration and fracture to right arm/elbow.
