An employee was pulling cow hides out of a 12-foot-deep hide pool using a long handled, stainless steel hook. The employee was pulling the hides to the edge of the pool, locating the hole in the hide, and hanging the hide on a hide-chain hook for it to travel to the folding table. One end of the long hook got caught on the forward chain hook. Then quickly caught the second hide-chain hook. As the chain hooks got farther apart, the two hooks began to pull the long hook. The employee was attempting to pull the long hook out of the two hooks when their right thumb was caught in a pinch point, resulting in amputation.

An employee was waiting to off-load a truck that was being backed up to the door when the dock plate malfunctioned. The employee was helping to lower the dock plate manually when the plate fell and broke the employee's left arm.

An employee was cutting meat using a buster saw. The employee's fingers were caught between the blade and the guide, and the blade amputated three fingers.

An employee was using a bandsaw. The product slipped out of the employee's hand and the bandsaw amputated the employee's right thumb, also breaking the index finger and lacerating the middle finger.

An employee had just pushed meat across the roller and blade of a skinning machine when the roller grabbed the employee's skinning glove and pulled the employee's right index finger into the blade, lacerating it.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was using a de-clipping machine to cut the metal clips off of sausage sticks. As the employee put a link of sausages into the machine, their hand slipped into the point of operation. A piston pinched the employee's left index finger, causing a partial amputation.

An employee was operating a small tabletop grinder in a food safety quality assurance lab. She was stuffing meat down the grinder's chute when the grinder caught her right hand. Its auger caused an amputation to her middle finger and a partial amputation to her index finger.

An employee was cleaning a part on a pedestal grinder. The employee's middle finger contacted the abrasive wheel, resulting in a partial amputation.

A maintenance employee was removing an inspection plate from an industrial cooker. Hot liquid spilled onto him and burned his hands, arms, and torso area. He was hospitalized.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
