An employee was standing on an anti-fatigue mat retrieving items from a conveyor when she tripped over the edge of the mat and fell. She sustained a hip fracture.
An employee turned a corner while walking, tripped over the corner of a pallet on the floor, fell onto her arm, and broke it. She was hospitalized, requiring surgery.
An employee was working at an extrusion line and monitoring pipe as it came out of the extrusion machine. The employee went to prevent the production of a defective part and his hand passed through a sensor that triggered the action point for the machine. The machine's clamp came down to pick up the formed pipe and it fractured the employee's arm.
An employee was comparing the lengths of two magnetic servo rods. The rods came together and pinched the employee's right index fingertip, causing a partial amputation.
An employee was attempting to unjam a machine when the hold down clamp activated and crushed his right ring finger, resulting in amputation below the fingernail. The machine was not locked out or tagged out at the time.
