The employee went to prevent the production of a defective part and his hand passed through a sensor that triggered the action point for the machine. The machine's clamp came down to pick up the formed pipe and it fractured the employee's arm.

An employee was comparing the lengths of two magnetic servo rods. The rods came together and pinched the employee's right index fingertip, causing a partial amputation.
