An employee was moving extra wire out of a cleaning tank by using the tank's jogging function. Two of the employee's right-hand fingers were caught on the last strand going around the acid roll, and the employee suffered an amputation to the right little fingertip.

An employee was walking on a level office floor. The employee tripped on a sticky spot (where something had spilled and dried), fell to the floor and landed on their left hip, and suffered a broken left femur.

An employee was on top of an automated weigh scale cart when they became caught between a weigh tub that was in motion and a chemical feed tube, resulting in a broken pelvis requiring surgery and multiple fractures to the spine.

On April 19, 2024, an employee was using a vertical belt sander to sand a burr off a piece of metal. The piece of metal got caught between the belt and the rest of the table and pulled the employee's left index finger into the sanding belt resulting in partial amputation of the finger.

An employee was obtaining a sample from a mixing machine in the chemical department when their hand contacted the mixing blade. The employee sustained partial fingertip amputations to the middle and ring ringers. The machine was running at the time.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

An employee was moving metal out of a cart. He had removed one piece and was working to move another when the metal shifted. His right ring finger was crushed between the two pieces, and he suffered a partial amputation to the finger pad at the tip.

An employee was at the loading dock working to manually move an oversized load off of a pallet that was sitting on another pallet. The oversized load (a gas cylinder cradle) tipped over and crushed the employee's right leg against the ground. The employee sustained a fracture to their ankle/leg requiring surgery.

An employee was depressurizing a line stop actuator when their left arm was struck by a threaded, 2-inch nipple from the side of the line stop housing. The employee sustained fractures to their left arm and a rib.

An employee was cutting the top of an empty scrap barrel that previously contained antifreeze. The bottom of the barrel blew out due to pressure that built up and caused his jeans to catch fire, resulting in burns.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
