An employee was changing blades on a shear when his left hand was caught in the blade holder, amputating part of his thumb.

An employee was helping to add alloy to steel when a splash of alloy came through the door and hit his pant leg. He was kneeling at the time, so the splash ran down into his boot, causing burns.

An employee was performing maintenance on a crane. He removed a pin that retained the dead end of the wire rope. His finger entered the hole where the pin was and the rope shifted causing an amputation of his right index fingertip.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

An employee was operating a uni-axle press when she suffered a fingertip amputation.

An employee was wearing cut-resistant gloves while operating an automatic slitter. The machine stalled and the employee placed their right hand on a panel to clean it before cutting. The slitter started running again, catching and damaging the cut-resistant glove. The employee's hand made contact with the slitting blades, resulting in amputation of the right ring finger and injury to two other fingers and lacerations to the hand. The employee was hospitalized. The machine was not guarded at the time.

An employee slipped on ice in a parking lot and fell, suffering five broken ribs. The employee was hospitalized.

An employee was working at the carve station when his left middle finger was caught in the pinch point between the rollers and the wheel/steel plate resulting in a fingertip amputation.

An employee was unjamming a bucket conveyor when it moved and pinched his right ring finger.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
