An employee's hand was caught in a conveyor belt as they worked to clear a jam in a piece of equipment. The employee suffered a dislocation to the right elbow, as well as a broken ulna near the elbow, and was hospitalized.

An electrician was on a 9-foot scaffold performing valve maintenance on an 8-inch line near a heat exchanger. The electrician was working to cycle a valve that failed to close. Due to a misaligned drain line and an open manual valve, hot liquid dextrose (~250 F) was released onto the floor below, creating a steam cloud. The electrician was evacuating the scaffold using the scaffold ladder when they slipped and fell 5-6 feet into the hot liquid, sustaining first- and second-degree burns to multiple parts of their body including both arms, both hands, their back, and their buttocks.

An employee was remotely operating a scissor lift out of a low-ceiling garage when the right front wheel of the lift ran over the heel of his left foot, causing soft tissue damage. The employee was hospitalized and required surgery.

An employee was performing janitorial duties when their finger became caught between a full-sized dumpster and a metal pipe on the wall. The employee sustained a fingertip amputation.

An employee was doing grounds maintenance with a tractor, and was out of the tractor when it rolled, pinning the employee against a steel fence post. The employee was hospitalized with five broken ribs and right arm injuries.

An employee's left bicep was lacerated by high-pressure water during water blasting operations. The employee was hospitalized.
