An employee was standing on the platform at the galvanizer machine holding onto the metal hand rail and observing the moving shovel/ladle for cleaning the galvanizer. The shovel/ladle struck the middle divider of the holding tank causing the shovel/ladle to move up and down roughly. As a result the employee's hand was struck-by the handle of the shovel/ladle and their fingertip was caught between the metal hand rail and the metal handle and amputated.

An employee was backing up from a trailer on his forklift when the pallets fell off the forks. He got off to place the pallets back on the forks. The forklift rolled forward and pinned him between the pallets on the forks and the I-beam under the trailer. He suffered a broken leg and ankle and was hospitalized.

On December 30, 2019, an employee was beginning to thread a write through a machine when the employee stepped into a cooling tank, suffering a leg burn that required hospitalization.

An employee was walking in an aisle when a forklift backed up over her foot, resulting in a skin injury that required surgery to repair.

An employee was looking into the bucket of a front-end loader to see how much material was in it. A skid steer backed into the employee, who then fell into the edge of the front loader's bucket and suffered internal bleeding in the abdominal area.

An employee was spotting for a forklift operation. The forklift backed over the employee, who suffered a complete amputation to one lower leg and a partial amputation to the other leg. The employee was hospitalized.

An employee was cleaning his work area and preparing it to begin making air-set molds when a nearby forklift that was backing out from a load swung around and struck the employee. They sustained a lower left leg fracture.

An employee was walking around a work area when he was struck by a forklift that was backing up, resulting in several broken bones in his foot.

An employee was working to unjam an auger screw. The screw caught his glove, resulting in a severe open fracture to his left lower forearm. He was hospitalized, requiring surgery.

An employee was working in a warehouse without air conditioning on a day when the temperature outside was in the 90s. The employee developed heat stress, became ill, and was hospitalized.

An employee was scraping down the sides of a mixing machine. The machine activated and her right arm was caught in the auger, resulting in a fractured lower arm.

Two employees were conducting a roll change on a rolling mill machine using a porter bar and a counterweight sleeve. The sleeve slid back and crushed the injured employee's right index fingertip between the sleeve and the counterweight, resulting in a partial amputation.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
