Two employees had just finished placing signs on a sidewalk. A vehicle jumped the curb and struck both employees. Both were hospitalized; one suffered fractures in the hand, elbow, fingers, and lower back, as well as a shoulder injury.

An employee had responded to a downed light pole outside of the building and possibly sustained an electric shock.

An employee was walking down metal stairs. The employee's ice cleats got caught on the stairs and the employee fell back against the stairs, suffering two broken vertebrae.

An employee was performing preventive maintenance on conveyor belts and checking a malfunctioning belt when his finger was caught between the belt and the conveyor frame, amputating his fingertip. The conveyor was running at the time.

An employee was working on a lighting issue while standing on a step ladder. As he climbed down the ladder, he missed the last rung and fell to the pavement, hitting his head and suffering a concussion that required hospitalization.

An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.

An employee was using a utility knife to cut honeycomb paper when the knife amputated their left index fingertip.

An employee was cleaning a resident's chimney. While using a drill, the employee's glove became caught in the drill and their finger was fractured.

An employee was grinding some hardfacing on a bulldozer blade when the 9-inch grinder kicked back and lacerated his left leg above the knee. The employee was hospitalized and required surgery.

An employee was operating a rotating tool to place thread protectors on the end of pipe. Her left hand was on the rotating part of the tool when the tool engaged, resulting in a left forearm fracture and a thumb laceration.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
