An employee was loading material into a die cutter when her finger was caught in the gripper at a nip point, resulting in an amputated left ring fingertip.

An employee was cleaning a copy machine. While trying to open the filter compartment to finish the cleaning by rinsing with water, the pressure exerted on the lid caused a solution with possible traces of a sodium hypochlorite to spray into the employee's eyes, resulting in eye irritation that required hospitalization.

An employee was priming the seal water pumps for a paper machine. The employee removed a cap and was struck by discharging hot water, suffering first- and second-degree burns to the arms and back. The employee was hospitalized.

On or about September 22, 2016, an employee was performing maintenance on a converting machine that makes corrugated boxes when the hinged door swung shut, partially amputating his right ring finger to the distal knuckle.

An employee was feeding the prefeeder when he stepped in a hole that was created by a missing roller. He suffered a swollen knee and was hospitalized.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Tools and parts were being lowered to the ground from an elevated platform in a 5-gallon bucket. The rope being used to lower the bucket failed; the tools and parts fell and lacerated the left forearm and upper left leg of an employee on the ground.

An employee was tracking an infeed belt on a scrap hogger when their right arm was caught between the belt and a roller. The arm was broken and the employee's shoulder was dislocated. The employee was hospitalized.

An employee was working to clear a jam from a paper napkin folder. The ram that advanced the napkins pushed the employee's right arm into the side of the machine, causing several lacerations and a hairline fracture.

An employee was troubleshooting overflowing ink on a printer-slotter machine when their right ring fingertip was caught in a guarded roller, resulting in a partial amputation.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
