An employee was using a utility knife to cut a spool roll to get excess paper off the spool. The employee then tripped with the knife in their hand and fell. The utility knife cut through the employee's glove and partially amputated their left middle finger halfway through the nailbed.

Georgia-Pacific Savannah River, LLC

An employee was observing contractors lower a 44-by-88-by- 1/4-inch housing lid using chain fall hoist rigging. The lid came to rest on the guides and was not fitting into place. The employee was pointing and his finger was caught between the lid and housing when the guard was pushed into place. The employee's right index fingertip was amputated.

An employee was unclogging a drain line from the tertiary screen in the recycle mill operations section. While lifting the basket out to clean, hot water sprayed through the seams of the filter.
