An employee was operating a pneumatic chop saw when his hand was struck by the blade, resulting in the amputation of all four left hand fingers at the lowest knuckle.

An employee was injured on January 13, 2016, while adjusting a three-phase unit that was loaded onto a conveyor. A piece of the welding wire grazed her right thigh. An infection developed in the thigh that later required inpatient hospitalization.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

An employee was troubleshooting a machine. He was investigating a slipping belt when it caught his left hand and pulled it into a pulley, partially amputating the tip of his thumb.

An employee was tilting a radiator in a fixture. The radiator moved in the fixture and crushed the employee's left index fingertip, causing an amputation.

An employee installed a busbar on a coil for a winding machine. The busbar hit a tension bar, causing his left ring finger to get caught between the coil and the tension bar, which pulled in the employee's glove and resulted in a medical fingertip amputation. The machine was guarded at the time.

An employee was operating a wire winding machine when their clothing was caught in a rotating component, pulling them into the equipment. The employee's right arm was fractured.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
