An employee was using a forklift to move empty plastic totes that were stacked three levels high. As the employee was driving the forklift toward the bottom pallet, the totes were pushed against the middle bar of the material rack. This caused the plastic totes to fall onto the overhead guard of the forklift where the employee's hand was resting. The employee sustained a right middle finger fracture.
