An employee was removing a pallet of faucets from a bay area when a shrink-wrapped pallet containing approximately 25 air conditioning units fell and struck the employee. The employee sustained multiple injuries to various parts of the body and was hospitalized.

On February 7, 2024, an employee was driving a forklift when it struck a pallet of lumber. Her foot was pinned between the forklift and the lumber and was fractured.

An employee was on a storage rack moving merchandise when they lost their balance and fell to the floor, resulting in a concussion.

An employee was spotting for a piece of lifting equipment. Another piece of lifting equipment ran over the employee's left foot and broke it.

An employee was working on a three-step ladder resetting vacuum displays when he fell on his back to the floor, striking and injuring his head. He was hospitalized.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

The injured employee was walking through the warehouse. Another associate was driving a forklift with a pallet of large format tile (slabs) when the slabs began to tip. The injured employee ran over to stop the slabs from falling, but the slabs fell on his lower left leg/foot. The employee sustained fractures in his lower leg and foot. He was hospitalized and required surgery.

An employee was doing a cutback on a piece of lumber using a chop saw. The saw hit a knot, the piece of lumber buckled, then kicked back and broke the kickback guard. The board was then pulled back into the blade, which lacerated the employee's left index and middle fingers.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was descending a rolling ladder when she fell from the last step and her head struck the floor, resulting in a fractured pelvis.

On September 24, 2025, an employee was using a sledgehammer to straighten the forks on a forklift when a piece of metal broke off the fork and became lodged in the employee's leg. The employee was hospitalized.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
