An employee was tightening a bolt on the final assembly of a stump puller. The wheels were separated and resting on 6-inch steel blocks with the mainframe on steel I-beams. The employee stepped between the wheels to adjust the spacer plates and used his legs to separate the wheels. When the stump puller was raised slightly and the weight came off the support beams and blocks, it rolled backward a few inches, causing the lifting wheels to close and catch his left leg between the wheels. The employee sustained a left tibia fracture, a torn calf, and bruising to the lower extremities.

An employee was cleaning a hitch pin on a manual lathe using a terrycloth rag. The rag was pulled into the spinning three-jaw chuck, pulling his left arm into the machine. The employee sustained an open left arm fracture three inches above the wrist.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

An excavator bucket was being removed. An employee was pulling a snap ring to remove a pin and lost his grip. The high-tension snap ring recoiled and his left middle fingertip was amputated by the snap ring.

An employee was walking between service bays. He came into contact with a trailer hitch, which was sticking out of the back of a lawn mower, and fell to the concrete floor. He landed on his right side and suffered a broken hip.

An employee was taking a scissor lift off a flatbed trailer. While using the remote operator controls and walking alongside the lift, the wheel of the lift turned in the wrong direction, causing it to strike the employee and roll over his left ankle. The employee was hospitalized with two broken bones in his ankle.

An employee was working on an excavator's tracks. He was swinging a sledgehammer to finish pushing a metal tractor pin into the track links. A hammer-blow caused a piece of the pin to fly off, ricochet, and embed itself in his buttock. He was hospitalized, requiring surgery.

An employee was picking up crane pads to put onto their trailer. Their finger got pinched between pads, resulting in an amputation involving the loss of skin.
