Final narrative
On April 12, 2023, an employee was pulling a stack of metal connecting sheets off the forks of a skid steer and stacking them on a trailer. After pulling a stack of 6-7 sheets off, the sheets shifted and the metal edge went into the employee's left middle finger, resulting in an amputation above the top knuckle.

On October 5, 2024, a crew was staging the last string of railcars to be loaded when the injured employee stepped forward and tripped on a railroad tie, causing his right foot to be on the rail. A staged railcar rolled forward and caught his foot between the wheel and the rail. The employee was hospitalized for amputations to toes.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

The rail car moved and he ducked to avoid the grain spout, which snagged his fall protection, causing him to fall into the rail car. An employee was attempting to seal a leak at the strainer plug located on a pressurized anhydrous ammonia toolbar. The employee was tightening a plug on the strainer using a wrench when the plug failed and a burst of anhydrous ammonia hit the employee in the face and torso. The employee suffered chemical burns to the eyes and mouth as well as the left side of their head, neck, and shoulder.
