Final narrative
An employee was helping secure a support structure for a heat exchanger that was going to be cut. The support structure was cut and the heat exchanger put tension on the strap that was securing it. The strap then broke and the heat exchanger fell. It struck the employee causing a laceration to their left lower leg.

An employee was bending down working in a team to level a heat exchanger so it could be bolted to the floor. The employee's left little finger was lacerated and crushed as the equipment was lowered down; the employee was hospitalized.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
