An employee was working in the store s merchandise storage room unpacking totes of merchandise that had recently been delivered. While moving totes, the employee fell on the same level and sustained a fractured left hip requiring hospitalization.

An employee was receiving a delivery and unwrapping shrink wrap from totes on a pallet. Some totes fell out causing him to fall backward, and then additional items fell on him. The employee sustained injuries to his back and ribs.

An employee was removing outdated products from a stand-up pharmacy cooler. When the employee stood up, he lost balance and grabbed the door handle, causing the cooler to tip over. The employee fell backward onto the ground and the cooler fell on them. The employee was hospitalized with a fracture and a contusion.

An employee was helping a customer and placed a basket on the floor. As she continued to work, the employee tripped over the basket and fell, sustaining knee and femur fractures.

An employee tripped on a cord in an aisle, fell to the floor, and suffered two broken vertebrae. The employee was hospitalized.

Two employees were walking through the warehouse behind a forklift that was moving pallets of 30-foot metal bottom bars. When the forklift reversed, the top of the forklift struck a box of metal bottom bars on top of racking, causing the box to fall and strike the two employees. They were both taken to the hospital; one employee was treated and released without being admitted. The other employee was hospitalized with a fractured left orbital bone and an injured right knee.

The injured employee was spray painting lay flat shipping reels while another employee was picking up the reels with a telehandler after they were painted. The telehandler bumped a reel, causing other reels to fall. The injured employee was caught between the reels, resulting in broken left femur.

An employee was installing an aerial drop cable across a roadway. The employee was on a ladder positioned against a telephone pole. Cable had been installed on one side of the roadway. The employee was making the connection to the pole on the opposite side of the roadway when a box truck contacted the cable, causing the employee to fall onto the grass of the customer's yard. The employee sustained broken ribs and was hospitalized.

An employee was performing routine housekeeping duties when a bale of plastic bottles slid off the forks of a forklift that was backing out of a trailer. The bale struck the employee, pushing her into another machine. The employee suffered a fractured rib and punctured lung.

An employee was using a 4-by-4 to straighten a pack of wood. A forklift nudged the 4-by-4, which caused the employee's right hand to be caught between it and the pack of wood. The employee suffered a partial amputation to the right ring finger.

An employee was walking through the facility when they struck their foot against a pallet, tripped, fell into a pallet, and landed on the concrete floor. The employee's right leg was fractured.

A delivery driver was making a delivery at a client location. He was closing the rolling door of the delivery truck when his fingertip became caught in the door, resulting in a partial amputation.

An employee was servicing a rooftop HVAC unit when their left hand became caught in the rotating belt of the machine. The employee sustained an amputation of their thumb from the first knuckle.

An employee was descending a set of rolling stairs when he missed the last step and fell to the floor. The employee was hospitalized with injuries to his ankle and leg.

An employee was installing a new conveyor belt when their right thumb became caught between the belt and the drive pulley. The employee sustained a partial amputation of the thumb.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
