An employee was scraping the walls and ceiling of a barracks room in preparation for paint. The employee fell from the footboard of a bed to the floor below, resulting in leg fractures.

A firefighter was pulling an equipment tray from the side compartment of a fire truck. The tray came off its guide tracks and fell to the ground, amputating two fingertips on the firefighter's left hand in the process.

A police officer responded to an active shooter situation. He entered the building with three other base policemen and was shot in the leg by the suspect. He was hospitalized.

An employee slipped while walking, suffering fractures to the left arm and right leg.

On May 30, 2017, an employee was diving to a depth of 82-84 feet with a team in the Gulf of Mexico. He was using an oxygen tank, got tangled in the line and the air shut off. He was found unconscious by other divers and was brought to the surface were he was resuscitated.

An employee was driving a cart around the driving range to pick up golf balls. The vehicle struck an in-ground target causing the employee to strike his forehead on the windshield. The employee was hospitalized with fractures to their nose and cheekbone and lacerations to their head and hands.

An employee was operating a picker when it struck a stack of pallets. His body moved forward and he struck his face on the computer monitor in the vehicle, resulting in a concussion, laceration, and blurry vision.

An employee was driving a lift in reverse through an aisle while the lift was loaded with a headboard. The headboard caught merchandise in the aisle racks, causing it to shift and contact the employee's right leg. The employee suffered a broken leg.

An employee was driving a utility task vehicle (UTV) through the parking lot to the boat yard to meet a co-worker. The UTV and slid into a curb, causing it to flip on its side and land on the employee's leg, resulting in a broken leg.

On December 7, 2023, an employee was driving an order picker down an aisle when the mast of the vehicle struck a material pan that was hanging out of the rack. The pan fell and struck the employee, resulting in rib fractures and a spleen laceration.

An employee was activating the sprinkler system at a sports complex following recent maintenance to repair leaky pumps. When the system was activated, a section of newly installed 3-inch schedule 80 PVC pipe struck the employee, who lost consciousness. The employee suffered a broken jaw and a concussion.

An employee was ascending a flight of stairs after exiting their vehicle in an adjacent employee parking lot. The employee stepped to the right to avoid another employee on the left who was descending the stairs. The employee lost their balance and fell on the stairs. They were hospitalized with a laceration/abrasion to their chin, a fractured left humerus, and bruising to their left knee.

An employee was working on an electrical panel in a mechanical room when an arc flash occurred. The employee suffered burns.

At about 3:10 p.m. on October 3, 2025, an employee was inspecting a car. Two dogs that had been in another car were leashed and tethered to a bollard. As the employee inspected the first car along with a narcotics detection dog, one of the other dogs came loose and attacked the narcotics dog. The employee was separating the dogs when the other dog bit his left ring finger. The last joint of the finger was injured and part of it was bitten off.

An employee was climbing a fixed ladder to perform maintenance in an attic. While unlocking the attic entry, the employee fell from the ladder more than 10 feet to the concrete below. The employee suffered a fractured hip, pelvis, tail bone, arm, and elbow, resulting in hospitalization and surgery.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
