An employee was loading a trailer of automotive parts when the load caught the top of the trailer. He jumped up to clear a wedged item. When the item came down, the load collapsed onto the employee, resulting in a fractured pelvis.

An employee was performing maintenance on an a conveyor system. He was working to loosen the chain that moves the belt when his right index fingertip was caught by the chain and amputated.

An employee was walking on the dock when a forklift ran over their right foot. Their foot was crushed. The employee was taken hospitalized and required surgery.

An employee was cutting corner boards off a pallet with a box cutter. The box cutter lacerated his arm.

An employee was collecting pallets in the freezer when his foot slipped on condensation on the platform of the pallet jack and struck a post, resulting in a foot fracture.

On October 25, 2023, an employee was in the warehouse driving a forklift toward the dock door. The forklift struck a conveyor belt and the employee sustained broken ribs.

An employee was operating an electric pallet jack when it struck a stanchion. The employee sustained fractures to the right chest and right ankle.

An employee was operating a forklift when the vehicle collided with a wall. His right foot struck the wall, resulting in three fractures in his foot.

An employee was using a walkie-rider to load a trailer. As she turned a corner, the walkie-rider struck the trailer and the employee was thrown to the ground. She was hospitalized with a fractured left leg.

An employee was driving a forklift and struck a pole, resulting in a broken left ankle.

An employee was operating a rider pallet jack when it struck a stand-up forklift. The forks struck both of the employee's ankles, resulting in hospitalization.

An employee was working to repair a machine with a broken metal stopper. She removed the stopper and used a roll of tape to temporarily secure it. While she was moving it back into place to fix the machine, the residual pressure of the plastic pallets forced the stopper down, resulting in her right thumb becoming stuck between two metal parts. The thumb was amputated.

An employee was changing the battery on his forklift. He removed the forklift's battery plate and exited the battery machine alignment track. The shift lead then went to retrieve a new battery. The employee entered the battery machine alignment track again to unplug the charging cable. The battery changing equipment proceeded forward and pinned the employee against the forklift. The employee was hospitalized with two fractured ribs.

An employee took a picture of the seal and license plate on a delivery truck after loading. The truck then reversed back to the dock and pinned the injured employee against the dock. The employee sustained an abdominal fracture.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
