On September 11, 2024, an employee was moving between a stack of steel and a hopper when they tripped and fell to the ground. The employee was hospitalized due to a fractured and displaced hip.

At about 7:55 a.m., on January 29, 2024, an employee's left leg struck the hitch of a parked tugger as they were passing by. The employee suffered a displaced fracture to the leg, as well as a dislocated ankle. They were hospitalized and underwent surgery.

On April 27, 2023, at 7:05 AM, a machine operator was removing a stuck part from a pneumatic die tool and amputated their left middle fingertip.

An employee was performing a die change on a 150-ton press when he began to feel weak, suffering from possible dehydration and/or heat exhaustion.

An employee was on an extension ladder, changing a ballast. The employee fell from the ladder to the workroom floor about 12 feet below, suffering three broken ribs.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was cutting angle stock on a band saw when the blade amputated their thumb.

An employee was loading a press machine prior to pressing a two-handed device. The two-handed device malfunctioned and the press cycled while the employee's finger was in the point of operation. The employee sustained the amputation of the first digit of the left ring finger.

An employee pulled a 5-pound box from a stack and tripped on shrink wrap, suffering a broken hip.

An employee was running a milling machine when a software glitch allowed the machine's clamp to close on and amputate the employee's fingertip.

An employee was removing a solidified plastic plug from the plastic injection port of a molding machine when pieces of hot plastic contacted the employee's skin, causing thermal burns to the right forearm.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
