An employee was wiping sand piles off the grate of a pump machine when his right hand slipped and his middle and ring fingers entered the grate. The employee suffered amputation of the middle fingertip as well as a severed ring finger that was reattached.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
