An employee was opening a valve to allow gas to empty into a manifold system when the line blew off. The employee was struck by the barbed hose from a tank, resulting in a laceration on the left side of the head between the left eye and ear as well as a neck fracture.

An employee was changing the tooling on a press brake when the machine was activated via the foot pedal. This caused the machine die to crush and break his left middle and index fingers. He was hospitalized and had surgery.
An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.
