Final narrative
An employee was operating an electric pallet jack. The pallet jack collided with a forklift, and the employee's foot was caught between the carriage of the pallet jack and the rear of the forklift. The employee's left big toe was amputated.
On May 21, 2024, an employee was operating a pallet jack. After exiting a cooler, the employee went to turn the pallet jack around and struck a rail. The employee sustained a broken right leg.
