Final narrative
On May 19, 2023, an employee was walking away from a pallet when his foot became caught in a piece of plastic banding. The employee tripped and fell to the floor, resulting in a hip fracture.
An employee was removing a foreign object from a surge tank on the line when their right hand contacted the agitator wheel. The employee's thumb was fractured and lacerated.
On September 24, 2022, two employees were cleaning an aseptic processor room. When a stainless steel filter housing lid clamp was removed, sodium hydroxide solution heated to 170 degrees escaped and sprayed out onto the two employees. One of them suffered second-degree burns to the arms and shoulder.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was preparing for a clean-in-place (CIP) process and removed a cap from a pipe to connect another pipe. Hot water (approximately 170 degrees) splashed onto the employee, causing burns to her hands, left forearm, and both thighs.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
