An employee was preparing for a clean-in-place (CIP) process and removed a cap from a pipe to connect another pipe. Hot water (approximately 170 degrees) splashed onto the employee, causing burns to her hands, left forearm, and both thighs.

An employee was removing and cleaning the water filter for a cheese cooker. The employee switched over to the alternate filter and used the shutoff valve to isolate the filter before removing it. When the filter was isolated, the hot water/steam mixture built back pressure in the filter housing. When the employee removed the clamp, the hot water burst out onto the employee causing second-degree burns to their inner arms and the left side of their chest and abdomen.

An employee was replacing the valve stem on a pneumatic valve when the valve actuated and closed on their thumb, resulting in a fingertip amputation.

An employee accessed an electrical panel to switch a breaker when they received a 480-volt electrical shock that went from their right hand to their left hand, resulting in hospitalization.

An employee was cleaning up in the packaging room when an alarm went off on the cheese knife. He went to turn around and tripped on a hose and fell backward to the floor. The employee was hospitalized with a fractured right hip.

An employee pulled out a bucket of hot oil from under the fryer. The employee then stood on the table to clean the back wall. He stepped down into the bucket of hot oil, resulting in third-degree burns to his left leg.

An employee was performing maintenance under a kettle. When he removed a tri-clamp on a transfer line, the hot fat and broth material in the kettle poured out onto his arm. The employee was hospitalized with burns.

After completing a run with an oil distributor truck, an employee was working to return (suction) the hot oil to the tank of the truck. When the employee opened one of the valves, hot oil (approximately 385 degrees) sprayed their face, resulting in first-, second-, and third-degree burns.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was using a water hose to clean debris out of the outfeed of a log conditioning vat. Water began entering the vat from the adjoining vat through a void in the separation wall at the infeed of the vats. As the employee was exiting the vat he had been working in using the access opening at the outfeed, hot water exiting the access opening entered the top of his protective hip wader. It pooled at the bottom of the wader and burned his left foot and ankle.

An employee was cleaning off a pulley on a conveyor belt when the conveyor caught her right index finger, resulting in a fingertip amputation.

On March 2, 2025, an employee was walking to a maintenance shop when they slipped on a wet tile floor and fell, resulting in a fractured right wrist.

An employee was checking an inline magnet. The magnet was placed on a cart next to another magnet and the magnets moved toward each other. The employee's right ring fingertip was caught and amputated.

An employee was working on a blending line and connecting a rotary valve to the bagging. He attempted to clear some powder stuck on the rotary valve while the blades were moving, resulting in the amputation of his right middle fingertip.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
