An employee was repairing the drive motor of a carcass-carrying cart. The employee raised the hinged slide and secured it with the chain attached to the slide. He then raised the cart off the ground by using the winch on the cart so that he could place wooden blocks underneath it. When the employee lowered the cart onto the blocks, the chain holding the slide in the upward position loosened, causing the slide to fall. The employee's left index finger was caught between the slide and metal plate, resulting in the amputation of his fingertip.

An employee was working at the base of a chute that was dispensing processed pig feet into a container. The employee was retrieving the feet and packaging them in boxes. As she turned and stepped back to pick up an empty box, she stepped onto the edge of a tub under the chute, lost her balance, and fell to the concrete floor. Her left femur was broken near the hip. She was hospitalized.

An employee was fixing a mechanical casket lift in a vehicle when the lift came down on their left-hand fingers, resulting in amputation of two fingers.

An employee was helping to remove the clamp attachment paddles from a forklift so they could then repair a hydraulic leak. The clamp fell and pinned the employee's right ring finger against the mast. The employee's finger was partially amputated.

The employee slipped and as he went to catch himself, he accidentally engaged the up-down control for the forks. This caused the forks to fall onto his foot. An employee was in a scissor lift approximately 20 feet above the ground to place a plastic tarp on a catwalk in order to catch condensation. They positioned the lift opposite the work area and stepped out of the scissor lift onto an I-beam. The employee fell from the I-beam to the ground. The employee was hospitalized with a skull fracture on the right side of their head and injuries to the chest and both eyes.

A maintenance employee was called to look at a pallet conveyor machine where a chain had fallen off a sprocket wheel. When the employee grabbed the loose chain, a light sensor caused the machine to restart. This created a pinch point between the chain and sprocket that caught the employee's left little finger. The finger was surgically amputated at the second knuckle.

The injured employee was standing by one of the stationary computers along the wall. There was a half-pallet located approximately 3 feet behind the employee that consisted of 12-pound boxes stacked six boxes high. A forklift was transporting a pallet that came in contact with the stationary pallet behind the injured employee, causing four boxes to fall and contact their legs. The injured employee was hospitalized with leg injuries.
