An employee was making an adjustment to an extruder. The employee's gloved left hand was caught in a roll-to-roll nip point causing fractures to the ring and little fingers. The employee underwent a medical amputation to the little fingertip.

An employee was trying to manually unjam a component within a machine. An abrupt rotation within the machine caused the employee's left middle fingertip to be crushed between two metal machine parts. The fingertip was amputated.

An employee was unloading new equipment from a pallet skid. The employee was removing the strapping from a panel when it became dislodged from the transportation skid and began to tip. The employee attempted to balance the paneling but succumbed to the size. The panel continued to tip and caught the employee's finger between the panel and the parked forklift's mast, resulting in a partial amputation to the right ring finger from the nail bed to the tip.

An employee had just finished maintenance work on an HVAC system. He was closing the guard cover when it slipped, smashed his left hand (bruising two fingers), and severed the tip of the ring finger.

An employee was removing a plastic component that was jammed in a de-energized machine. The employee placed their left hand on a metal slide, then used pliers with their right hand to pry the slide open. Upon applying significant force to the slide with the pliers, the jam broke free. The employee's left index fingertip was pinched and amputated between the slide and an adjacent metal surface.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was shadowing coworkers while in training on the wide extrusion line. The employee went to clean the roller with a rag and cleaner when his right index finger got caught between a powered roller and a rubber roller. The employee sustained an amputation to a finger.

An employee was clearing a jam in a machine when it cycled. His hand was caught in the die, resulting in the amputation of two fingertips.

While operating a machine, an employee noticed that a piece of plastic material was stuck to a chrome roll. The employee was removing the plastic when their arm came into contact with the rollers. The employee suffered a contusion to the upper arm between the elbow and the shoulder.

On June 9, 2025, a machine operator was using a sorting machine to sort/clean plastic sheets. While she was freeing stuck sheets on the lift table, the table engaged and crushed her against the top railing, trapping her torso. The employee was hospitalized with contusions to her back.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
