On July 22, 2023, an employee was swinging a 5-ton hammer on a hammer union. The employee then noticed a piece of metal was lodged in his upper left thigh and required hospitalization.

An employee used a wire brush to clean the threads of two 10-foot joints of 2-inch figure 1502 iron pipe, which were sitting on the rack of a monorail trailer. As the employee rotated the pipe inward to clean the threads of the innermost joint, the opposite end of the pipe dislodged from the rack and fell to the ground. As it fell, the end nearest to the employee levered upward and pinched their right thumb between the two joints of pipe. The employee sustained a laceration and fracture to the thumb.

An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.
