Final narrative
On June 21, 2023, at approximately 11:45 a.m., an employee climbed a dock plate at a customer site to avoid heavy rain. While reaching with his left hand to support his footing, his gloved left hand contacted a powered conveyor chain that was next to the dock area. His left ring fingertip was medically amputated.
On April 23, 2024, at approximately 7:30 PM, a driver was picking up a trailer at a rail yard. The employee was using a crowbar and hammer to adjust the pins of the chassis to allow the intermodal container to sit on the chassis. The container shifted and the employee's left hand was caught between the container and chassis, resulting in a partial amputation of their middle, ring, and little fingers.
An employee had pulled his semi-trailer over at a rest stop because the trailer door had opened. He was trying to pull down the door, gripping a strap with one hand and the trailer grip handle with the other hand, when he fell to the ground 2-3 feet below. He suffered a broken right shoulder, a fracture to a right-side middle rib, a punctured right lung, and a swollen esophagus.
An employee was struck by a large paper roll upon opening semi-trailer doors after the load shifted during transport. The employee sustained a fractured right leg and left toes.
An employee was attempting to pull a fifth wheel below a semi-trailer. It was tight, so he grabbed a puller to use on the wheel. In the process, the puller slipped and the employee fell to the ground on his right side, resulting in a fractured right hip that required surgery.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.
An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.
An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.
An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.
An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.
An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.
A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.
After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
