An employee was cleaning a conveyor belt when the gears on the belt caused a fingertip amputation.

An employee was closing an aircraft door when their right hand slipped and was caught in the door, resulting in a little finger amputation.

An employee tripped over a box, fell to the floor, and suffered a pelvic fracture.

On March 1, 2020, at approximately 5:40 a.m., the injured employee was catering a flight with employees 2 and 3. While the catering box on the lift truck was elevated, employees 2 and 3 were closing the aircraft's door and heard a noise. They observed the injured employee on the ground below. It is believed that the injured employee was inside the catering box while it was raised to service the aircraft and fell out after the box's door was opened. The injured employee sustained a skull fracture.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.

An employee was cleaning a steam kettle; he had the floor grate lifted to access the drain area. He tripped on the open floor grate, striking his knee against the steam kettle's drain pipe and falling onto multiple pieces of kitchen equipment. He landed on the floor, where he struck his head. He was hospitalized with lacerations to his ear, which required surgery to repair.

An employee was outside a freight elevator, reaching in, when the elevator moved and crushed her right hand. She suffered a broken arm and wrist and was hospitalized, requiring muscle surgery.

On September 29, 2025, an employee was removing a pan of pasta from the stove when the pan tipped over. Hot water contacted her legs and feet, resulting in burns to both feet that required hospitalization.

An employee was walking through a passageway sideways when the edge of her shoe got caught and she fell onto her right hip, resulting in a broken femoral head.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
