An employee was walking around a poly cart to attach the hitch to another cart when she tripped on a piece of nylon twine that was tied to the poly cart. The employee fell to the concrete floor and sustained a displaced femoral neck fracture of the left hip.

An employee was evaluating a coiling machine to verify that the blade was present inside it. The machine's weld function was activated, and the machine crushed the employee's fingers, resulting in the amputation of the left middle and ring fingers.
An employee was changing the bit on a router. The machine was cycled and the bit struck the employee, causing a fracture and a puncture wound to the right foot. The router was not locked out at the time.
On July 27, 2020, an employee was operating an end rider pallet jack and struck an elevated platform. The employee sustained a laceration to the left calf.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.
The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.
