On December 16, 2024, at approximately 8:30 AM, an employee was replacing an air solenoid for a robot while on a three-step portable ladder. He slipped and fell to the concrete floor sustaining fractures to his left hip and left femur.

An employee was running a press saw. The employee's left middle finger became stuck in the clamp of the machine and was pulled into the saw blade, which amputated it.

An employee was trying to free a stuck mold from a blow mold machine. The mold closed on his right hand, crushing it. His index finger had to be amputated during surgery.

On 1/17/2019, an employee was walking through the production area when he was struck by a powered industrial vehicle while crossing an intersection. The employee was hospitalized with a broken foot and amputated toe(s).

An employee operating a press machine for 45 minutes was hospitalized from heat stress.

An employee was walking in an aisle when a forklift backed up over her foot, resulting in a skin injury that required surgery to repair.

He was working to remove insulation around a valve when either the valve or the line failed, and hot condensate contacted his right arm, right abdomen area, and the front of both legs, resulting in second- and third-degree burns.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
