An employee was on a ladder trying to make a pipe connection involving a suspended pipe. The hanger holding the pipe failed; the pipe fell and hit the ladder, which caused the employee to fall to the floor. The employee suffered a broken pelvis.

An employee reached for a band saw hanging on the wall when the weight of the tool caused the trigger to be pressed. When it activated, it cut through his gloves to the bone.

An employee was installing shelving. As they were sliding a metal pipe into the wooden shelving unit, the employee's hand was caught between the shelf support and the end of the pipe, resulting in a right index and middle finger fracture.

An employee was assembling a baker type scaffold. He was beginning to transverse down the scaffold, approximately six feet off the ground, when the scaffold tipped over and he fell. The employee was hospitalized with a fractured femur.
