The injured employee was inspecting a facility and was in the operating area. Another employee, who was operating material handling equipment nearby, picked up a furnace to move it. As the vehicle released the object, it struck the injured employee causing fractures including broken ribs, and internal injuries to the spleen, kidney, and liver that required hospitalization.

A material handler operator was moving recycling material. When the operator let go of the material, the grapple swung back in the other direction. A piece of pipe flew from the grapple, striking an employee on the back of her hard hat. The employee fell to the ground and lost consciousness. The employee sustained a right parietal scalp injury, frontal brain hemorrhage, and T1 and T3 spinous process fractures. The employee was hospitalized.

An employee tripped on a single step and fell. The employee suffered a broken ankle that required surgery.

An employee was climbing down between a truck and trailer to remove the tarp from the load when the material got caught on the tarp. As he was climbing down to remove the tarp from the material, he tripped on a pipe or bungee cord and fell from the fender to the ground. The employee sustained fractured ribs and a pneumothorax on the right side of his body.

A temporary employee was working in the copper mill watching the grinding machine to ensure it did not jam up with excess material. The employee was manually removing the excess material by reaching over the bar in front of the silo while the guard was removed when their glove and hand became caught in the grinding machine's rotor blades. The employee sustained a partial amputation of their right index finger.

An employee was operating an excavator and lifting a rail when it slipped through the grappler attachment and slid toward the excavator cab. The rail penetrated the glass and impaled the employee in the abdomen. He sustained lacerations to the abdomen and injuries to internal organs.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

KP Building Products, Inc.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

BJ'S WHOLESALE CLUB, INC.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

International Steel and Counterweights LLC

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

Seneca Mechanical, LLC

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Heavy Weight Inc

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

A shear operator's hand and fingers were caught in a machine's point of operation. The operator's right index finger was crushed and was partially medically amputated.

An employee was operating a forklift. He parked on a ramp and got off the forklift to retrieve material, at which point the forklift rolled backward and ran over his leg. The leg was broken and he was hospitalized, requiring surgery.

An employee was using a shear to cut metal when the blade cut his left ring finger and amputated his left little finger.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.
