Final narrative
An employee was assisting a co-worker to remove an inner form. The injured employee got on his hands and knees and started to chip away at the concrete. When the pressure was removed, the 1,100-pound concrete product fell out of the form and landed on the employee. He sustained fractures to the spine and shoulder blade as well as a collapsed lung and a spleen injury.

An employee was forming a light pole foundation when a cable anchor failed, causing the concrete form to fall and pin the employee between the form and the wall. The employee was hospitalized with a fractured pelvis.
