An employee was speaking with another employee who was on a forklift. The other employee began moving the forklift, but it reversed instead of moving forward. The back wheel of the forklift struck the injured employee's left foot, causing her to fall to the ground. She suffered injuries to her left foot and right knee.

An employee was using a stepladder to pour seasoning mix into a mixer. The scoop fell into the machine; as the employee was retrieving it, the machine amputated his right ring finger to the second knuckle and lacerated his right little finger with tendon damage. The machine was not guarded at the time of the incident.

An employee was trying to grind corn using a table grinder. The grinder's jar came off; while the employee tried to reconnect it to the base, the grinder lacerated her right middle and ring fingers. The grinder was running at the time.

An employee used a stationary horizontal band saw to cut a piece of sheet metal about 15 feet long. When the employee went to move the piece of sheet metal, the end hit an adjacent portable band saw, causing the portable band saw to tip off its stand. The employee walked over to upright the tipped over portable band saw and it completely fell off the stand. As the portable band saw fell, the employee's finger was caught between the stand and the falling portable band saw, causing lacerations and crushing injuries to the left pinky finger, which required hospitalization and surgery.

An employee had just finished unloading material from a truck. He was descending using the truck's built-in ladder when he missed the last rung, fell to the ground, and broke his hip and left wrist.

An employee was using a handheld grinder to cut a bolt. The grinding wheel broke apart and a fragment struck the employee s right hand, causing a laceration to the palm near the little finger.
