An employee was speaking with another employee who was on a forklift. The other employee began moving the forklift, but it reversed instead of moving forward. The back wheel of the forklift struck the injured employee's left foot, causing her to fall to the ground. She suffered injuries to her left foot and right knee.

An employee was unjamming a slicer head when his hand was injured by a pulley. The employee was hospitalized.

An employee was using a stepladder to pour seasoning mix into a mixer. The scoop fell into the machine; as the employee was retrieving it, the machine amputated his right ring finger to the second knuckle and lacerated his right little finger with tendon damage.
