A cab operator got out of their cab to fix a log on the shaver machine that was crossed up. They walked down a catwalk and fell over a guardrail landing on concrete 20 feet below and sustaining a cracked pelvis and shoulder as well as a head contusion.

An employee was attempting to clear a jam on the back side of a shaver box when he stepped over the handrails and slipped, coming into contact with the shaver knives. His right foot was amputated and he was hospitalized. The shaver heads were not locked out at the time of the incident.

P&C Roofing, Inc.

An employee was transitioning from a ladder onto a roof when the ladder slipped and they fell approximately 20 feet to the ground. The employee was hospitalized with fractures to their pelvis and heel.

An employee was climbing a ladder to bring material onto a roof. The ladder slid and the employee fell 16 feet to the ground, resulting in a broken knee.

Caterpillar, Inc.

An employee was performing an inspection of a bridge crane while working from a 19-foot scissor lift. The employee was going to use a pendant to move the crane to access a different portion of the crane for visual inspection. The scissor lift guard rails were below the girder of the crane to avoid contact. The tow arm for the crane collectors contacted the guardrail of the scissor lift and caused it to tip over. The employee stayed inside the scissor lift basket as he fell to the floor. The employee sustained fractures to the right arm and hip.

An employee was on a forklift that was elevated approximately 20 feet and was auditing inventory. The forklift malfunctioned and the employee fell down to the surface below, causing them to sustain multiple fractures.

An employee was repairing a photo eye on the infeed roll case when they dropped a tool. The employee went to grab the tool and his fingers contacted the chain and sprocket of the infeed roll case. His fingers were pulled into the pinch point and the fingertips were crushed, resulting in amputation of one fingertip.

An employee was troubleshooting dispersing equipment. A belt and pulley system was energized, and the moving belt caught the employee's right ring finger. He suffered a partial amputation at the first knuckle, below the nail.

West Fraser, Inc.

An employee was cleaning the photo eye sensors of a piece of equipment between the saw that cuts lumber into lengths and boards. The employee stepped on a metal grate that gave way and fell 8-10 feet. The employee was able to remove himself from the conveyor. His tibia was fractured just below the knee.

An employee was working to get a log back onto a carriage. The employee came into contact with the running saw, which amputated their arm.

IDAHO FOREST GROUP, LLC

While refilling a lubricant tank, an employee observed that a pump was not operating correctly. While troubleshooting the issue, the employee reached toward the back of the pump to check for proper airflow. Their left index finger contacted moving components in the pump's piston mechanism, resulting in partial amputation of the fingertip.

GS II Building Products, Inc.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
