Final narrative
On August 17, 2023, an employee was transporting a 5-ton spreader bar with a forklift. While lowering the beam, it rotated and became hung up on a different beam. The employee got off the forklift and pushed the left end web of the bar. The loading strap came off the forklift fork, causing the beam to drop to the ground. His left hand was caught under the beam and his fingertips were amputated.
