An employee was checking the industrial waste stream as it discharged into a 4,000-gallon waste collection tank. The employee's right wrist was splashed with corrosive chemicals, resulting in hospitalization for a chemical burn.

INDUSTRIAL CHEMICALS, INC. (ALABAMA)

On November 27, 2023, an employee was emptying a 2-inch product line that transports sodium hydroxide liquid from a rail car to a 275-gallon tote tank. The nozzle came out of the tote and sprayed sodium hydroxide onto the employee's face. The employee was hospitalized with chemical burns to their face, mouth, and neck.

JOST CHEMICAL COMPANY, INC.

An employee was working with sulfuric acid as part of the production process. While transferring the chemical from a large container to a smaller container, it splashed on his body and hand, resulting in a chemical burn.

An employee knelt in wet concrete while performing work as a concrete finisher and sustained a chemical burn to the right shin. The employee was hospitalized and required surgery.

An employee was operating a mica disc punch press. While the employee was working to clear material from the press, it cycled and amputated the tips of their left thumb and index finger. The press was guarded at the time of the incident.

An employee was repositioning a wire that would not advance in a crimping machine when the machine closed on her right index finger. The employee sustained a fingertip amputation.

An employee was repairing a benchtop wire processing machine that is used to make wire harnesses for commercial applications. A guard had been removed in order to access the components. The employee was testing the "trigger" sensor of the machine and adjusting the blades when the machine actuated and a blade amputated part of his left index fingertip.

An employee was moving a large safe weighing approximately 625 lbs. with a dolly that was specially-designed to move the safe. The safe fell from the dolly, struck the ground, fell forward onto the employee, striking their right hip/pelvis area before rolling off the employee onto the floor. The employee sustained a right hip/pelvic fracture.

An employee was taking inventory of metal plate parts in a cardboard box when a part dropped, smashing her left middle finger between parts. One centimeter of the employee's left middle finger was amputated.
