An employee was throwing boxes into the baler and moving about the baler area when a forklift ran over their left foot and fractured it.

Two employees were walking back to the building after a short break outside. While on break they encountered a customer driving fast in the parking lot and told the driver to slow down. Three people from this vehicle followed the employees onto the sales floor and assaulted the employees. One employee sustained fractures to their nose and jaw which required hospitalization and surgery. The other employee sustained injuries but was not hospitalized.

An employee was working on the sales floor when his right foot was run over by a forklift. The employee sustained a fractured little toe.

On 6/17/24, an employee was working behind the food counter when they tripped and fell to the floor. The employee sustained a broken left hip and was hospitalized for surgery.

An employee had been pushing a cart in the warehouse and retrieving cases from a pickup location. The employee became ill and experienced nausea and dizziness, caused by heat exhaustion.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee was lifting a box of produce in the backroom area when she suffered pain in her lower back and was unable to stand, resulting in hospitalization.

An employee was reversing a pallet jack into a parking space when his leg was caught between the pallet jack and a railing post. The employee was hospitalized with leg and back injuries.

An employee was walking in the back room when she was struck by a display pallet of ketchup, causing her to fall to the ground. The employee was hospitalized with a fractured pelvis and abrasions on her left wrist and arm.

On October 5, 2025, an employee was stocking materials when their foot struck an apparel rack, causing them to fall and suffer a hip injury that required hospitalization.

An employee was cleaning the restroom when she slipped and fell to the ground, injuring her ribs on the right side. The employee was hospitalized.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
