Two employees were walking back to the building after a short break outside. While on break they encountered a customer driving fast in the parking lot and told the driver to slow down. Three people from this vehicle followed the employees onto the sales floor and assaulted the employees. One employee sustained fractures to their nose and jaw which required hospitalization and surgery. The other employee sustained injuries but was not hospitalized.

An employee was working on the sales floor when his right foot was run over by a forklift. The employee sustained a fractured little toe.

On 6/17/24, an employee was working behind the food counter when they tripped and fell to the floor. The employee sustained a broken left hip and was hospitalized for surgery.

An employee was retrieving work materials from the bar area. She then left the bar area and was struck by a forklift. The employee was hospitalized with fractures and lacerations to the left foot that required surgery.

An employee had been pushing a cart in the warehouse and retrieving cases from a pickup location. The employee became ill and experienced nausea and dizziness, caused by heat exhaustion.

An employee was painting a round module when a front-end loader ran over his right leg and crushed it.

An employee was operating a switch truck at a rail yard. He got out of the switch truck to unlock a container that was attached to a unit in front of him. The switch truck rolled and pinned the employee against the container. The employee sustained a fractured pelvis, a ruptured spleen, and a possible spinal cord injury.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was moving branches and other material when they were struck in the leg by the track of a skid-steer loader, resulting in an ankle and lower leg injury.

An employee was waiting for an operator to bring mail over to a mail sorting machine when she became pinned between the machine and a stack of pallets being pushed by a powered industrial truck (PIT). The employee suffered bruising and swelling on her hips, lower back, knees, and left side; a puncture wound to her left thigh from a machine screw; a crushed right hand with numbness and tingling; numbness to the left big toe; and a right wrist sprain.

An employee was picking an item in the picking area when a can of sweet peas fell onto his left ring finger, resulting in a partial amputation through the phalanx.

An employee was in the dairy cooler and bent down to get something for a customer. While standing up, the back of his head struck a top stock cart. The employee suffered a brain bleed and required surgery.

On June 5, 2025, an employee was pouring a bottle of antimicrobial produce wash when they inhaled the product and sustained irritation in their lungs and dizziness. The employee was hospitalized.

An employee was walking when their foot caught on the edge of a plate connected to a beam and they fell to the ground. The employee sustained a fractured femur.

An employee was preparing to make a delivery to a store when his hand was caught in the side lift gate of the trailer. The employee sustained tissue amputations of his middle and ring fingertips and an open fracture to the tuft of his ring finger.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
