Using a crane, an employee set a fuel tank down on jack stands. He unhooked the rigging on the north side of the tank. He turned around and stepped away from the tank while using the crane control to raise the hoist hooks. The rigging on the south end of the tank was still attached to the crane. The tank slid on the jack stands and struck the employee on the back of his legs and knocked him to the floor. The tank came to rest on his feet. The employee sustained bruised feet and a sprained tendon in the left foot.

An employee was cutting a weld out of a roof panel when the cutting wheel became pinched between two sections of metal. The grinder spun and kicked back toward the employee, its disk striking her in the face. She suffered a laceration across the nose and on both cheeks.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
