An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

On July 24, 2025, an employee was unloading stacks of empty, 96-gallon medical waste containers. A stack of 10 containers, which weighed approximately 460 pounds, fell on him. The employee was hospitalized with multiple head, face, and neck injuries and a back injury requiring surgery.

An employee was standing between a truck and a roll-off container that was being offloaded. The container came down on their left ankle. The employee was hospitalized with a laceration and fracture to the ankle.

An employee was loading a gaylord into a truck when the gaylord ripped, causing him to lose his balance and fall from the liftgate to the pavement. The employee was hospitalized with fractures to his left wrist and femur.

An employee was cleaning the back of a tractor trailer. The tractor trailer pulled away from the dock and the employee fell from the dock lock to the ground below, breaking their right femur.

An employee was preparing a load for the electric pallet jack he was operating. He was walking backward when he tripped and fell. The pallet jack struck his left leg, and he was hospitalized with a broken left fibula.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
