A crew was preparing to remove the capital form. The injured employee went to unscrew the formwork nut with a wrench and pipe bar. The wrench slipped on the nut, causing the employee's left little fingertip to become wedged between the wrench and the nut. The employee sustained an amputation to the finger.

An employee was guiding a crane onto crane mats when one of the leading transitional mats separated and struck the employee in the lower right leg. The employee sustained a fracture to the lower leg and was hospitalized.

JOHNSON BROS. CORPORATION, a Southland Company

The injured employee was assisting with backfilling an excavation. The employee was kneeling on the pipe with both knees and was holding a piece of plywood while their right hand sat on the second pipe next to the hole. While the excavator was backfilling the trench he was shaking the bucket to allow the dirt to come out. A piece of concrete fell out of the bucket and struck the back of the employee's right hand, causing a displaced fracture to the metacarpal bone of the index finger, a fracture to the metacarpal bone of the middle finger, and a laceration.

An employee was assisting with cleaning the distributor truck's tank. The distributor tank was still hot causing a rapid release of stems, burning the employee and causing them to fall from the tank to the flatbed work truck. The employee sustained burns to the face and hands, a head contusion, and a laceration above the right eye.

An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Don Martin Corporation

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
