An employee was running pipe into a hole on a rig floor. He was still running the other joint into the hole when the laydown machine shot the joint of pipe. The pipe hit the employee in the jaw and he fell backward, suffering a fractured cheekbone.

HT Energy LLC

Mechanics were straightening a ladder on the derrick in the shop using an acetylene torch and a 10-ton hydraulic jack. As the mechanics were extending the jack to straighten out the ladder, the cylinder on the jack was turning and the quick-connect fitting ended up against the footrail of the derrick. This caused the nipple to break and a mist of oil was released from the jack. The oil mist ignited resulting in a flash fire. The injured employee sustained second- to third-degree burns burns to the right hand, the left cheek, and the left side of their neck as well as burns to both arms. The employee was hospitalized.

A crew was running rods in the hole when the chain on the rod transfer broke and fell to the rig floor. The chain struck the injured employee on the hard hat and whipped underneath the hard hat and struck the employee above their nose, causing a fracture and laceration. The employee was hospitalized.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was in the yard area on a forklift pulling lumber. Inclement weather began and the employee stopped the forklift and was walking to a safe location when they were struck by flying debris from the high winds or tornado. The employee sustained a leg fracture.

LYCON, INC.

After reassembly work on a ready-mix truck, an employee turned the truck on to check for leaks. The employee was standing on the truck platform, about 3 feet off the ground, when he heard an air leak. As he was turning off the truck, a flying bolt from the plate assembly struck his forehead and broke his skull. He was hospitalized.

Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
