Cooperative Laundry

An employee was trying to retrieve a towel that was stuck in a machine when the machine lid fell on their head. The employee was hospitalized.

An employee was climbing a portable ladder to retrieve an item from the second level when she hit her head against a ceiling reinforcement. The employee sustained a concussion and lacerations.

An employee was underneath a construction loader truck while working on it. As the employee got up, they struck their head on the frame of the truck and were hospitalized overnight for a concussion.

An employee was squatting to inspect a dough mixer. She struck her head and lost her balance, causing her to grab onto a piece of machinery. The machine crushed her right index finger, resulting in an amputation.

An employee was setting a pipe in a trench about 3 feet deep. When he stood up, his head hit the wall of the trench and he suffered a neck injury.

An employee was straightening up after bending over. He hit his head on a ceiling beam and lost consciousness, having suffered a concussion.

An employee opened an office door, then moved away from it, at which point their left little finger was crushed in the door frame. The employee suffered an open wound to the finger with a nailbed injury, an exposed tendon, and a fracture, resulting in hospitalization.

An employee was walking when they slipped in some water but didn't fall to the ground. The employee was hospitalized with a broken blood vessel in his right calf.

On August 6, 2025, an employee was placing cardboard into a cardboard compactor when his right middle and index fingertips were amputated. The employee was hospitalized for hand surgery.

An employee was performing sheet pressing. The employee reached down to the right to get a sheet. The damp sheets were heavy and the employee placed his left arm on the lower presser. The employee reached to remove the cover from the bin to get a sheet when the unguarded switch was inadvertently activated causing the presser to actuate the upper press. The upper presser closed down on the employee's arm resulting in hospitalization with burns.

An employee was fixing a steam leak from a boiler. He used rope to secure the manhole cover while venting residual steam from the manhole opening. When he released tension on the rope, the manhole cover dropped into the tank and splashed hot water onto the employee. He sustained first- and second-degree burns to his right arm and first-degree burns to his upper chest.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
