An employee was servicing a tire on a garbage truck. The employee's left ring finger was caught between the tire and the impact wrench they were using, and the fingertip was partially amputated.

An employee was operating a vehicle with a roll-off dumpster. The employee was exposed to carbon monoxide from a leaking exhaust system. The employee was hospitalized for carbon monoxide exposure.

An employee was using a sledge hammer to straighten a piece of steel when he lost his balance, causing him to turn and twist ankle. The employee suffered a broken ankle that required hospitalization and surgery.

An employee was climbing the fixed ladder on a garbage truck when he fell approximately 5 feet, 8 inches to the ground. He sustained two rib fractures, abrasions to his face, and a spleen injury.

An employee was diagnosing a problem with the under carriage of a vehicle and tracing a leak. The airline came out of a fitting at the rear of the vehicle and quickly released pressure, causing the tag axle to fall or come down onto the employee's chest. The employee sustained a crush injury to the chest.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

A driver had backed up a rear load truck to a dumpster. The winch cable was bound, and another employee went to use the controls to put slack in the line. The cable tightened instead of loosening while the driver was holding it, and it caught his hand against the truck and crushed his thumb. The thumb was fractured and lacerated; he was hospitalized, requiring surgery. The incident took place during a training activity.

An employee left the guard shack to speak to a customer. A bulldozer ran over her right foot, resulting in three broken toes.

An employee was pulling into a customer's parking lot to pick up a roll-off container when the vehicle ran over the median curb. The vehicle then veered to the left, striking the side curb and the retaining wall. The employee sustained injuries to his neck and spine, as well as a damaged artery, a head laceration, and fractured ribs.

An employee was replacing the bracing arm of a baler. The replacement part (weighing an estimated 500 pounds) was moved into place using a truck-mounted crane with a lifting strap attached to the designated pick point. The employee then used his right hand to stabilize/align the part. The part shifted, crushing his right index finger against the mounting pin hole. The employee's finger was amputated above the first knuckle.

An employee was moving a mower with a tractor bucket when the bucket slipped and struck another employee on the hand. The injured employee sustained a partial amputation of their left little finger.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
